About Developmental Delays

Developmental delay refers to a child not reaching expected developmental milestones within the typical age range across one or more areas — including motor, speech, cognitive, social, and adaptive development. Global developmental delay (GDD) describes significant delay across two or more domains in children under 5 years. Motor delay is one of the most common presentations in paediatric physiotherapy, encompassing delayed gross motor skills such as rolling, sitting, crawling, standing, and walking. Early identification and intervention are strongly recommended by the NHS and NICE, as intervention during the critical early years of brain development has a significantly greater impact than later treatment.

Developmental delays affect approximately 1–3% of children in the UK. They may occur in isolation or in association with an underlying diagnosis such as a genetic condition, cerebral palsy, or autism. In some cases no underlying cause is found (idiopathic delay). Premature birth, birth complications, and environmental factors can all contribute.

Signs & Symptoms

  • Not achieving expected motor milestones — not sitting by 9 months, not walking by 18 months
  • Hypotonia (low muscle tone) — floppy appearance, difficulty supporting own weight
  • Asymmetrical movement or preferring one side of the body
  • Difficulty with floor play and transitions between positions
  • Poor head control in young babies
  • Delayed or absent crawling
  • Unusual walking patterns when walking is achieved
  • Difficulties with stairs, jumping, or balance activities
